What the Research Actually Found
The strongest summary comes from a 2021 review that pooled seven studies and roughly 386,000 people. It found a clear association between living in a hard-water area and a higher risk of eczema (Jabbar-Lopez et al., 2021). A large UK cohort sharpened the picture. Among 1,303 three-month-old infants, those in hard-water homes had a significantly higher rate of eczema than those in soft-water homes, and the risk climbed further when the water was also heavily chlorinated (Perkin et al., 2016). What gives the finding weight is its consistency: the same pattern appears across very different study designs. So does hard water cause eczema outright? No. But it raises the odds, and the harder the water, the higher they climb.
Why Hard Water Irritates Eczema-Prone Skin
Hard water, the first of those two stressors, irritates eczema-prone skin in two related ways: a residue it leaves on the surface, and a shift it causes in skin pH. Both matter most for skin that is already vulnerable.
The residue comes first. Calcium in hard water reacts with the surfactants in soap and body wash to form an insoluble film, the same scum that clouds shower glass. Instead of rinsing away, it settles on the skin. In a controlled study, skin washed in hard water was measurably drier, redder, and less hydrated than skin washed in mineral-free water, and the residue itself was the cause (Warren et al., 1996). On an eczema-prone barrier, that film is a daily irritant left exactly where the skin can least afford it.
The pH shift is the second. Healthy skin holds a slightly acidic surface, the acid mantle, which keeps its repair enzymes working and its defenses intact. Hard water is alkaline, so every wash pushes the surface out of that range and slows the barrier's ability to recover between flares. The two effects compound: the film irritates the skin while the pH shift weakens its capacity to heal. Why It Hits Some People and Not Others
Not everyone in a hard-water area develops eczema, and that puzzled researchers for years. The answer is largely genetic. About one in ten people carries a variant in a gene called filaggrin, which helps build the skin barrier, and that variant leaves the barrier weaker from birth. When researchers followed infants over three years, hard water on its own showed no overall effect on eczema rates. But among the children who carried the filaggrin variant, harder water roughly tripled the risk (Jabbar-Lopez et al., 2020). The lesson is not that hard water is harmless for everyone else. It is that the same exposure lands far harder on a barrier that started out compromised, which is precisely the barrier eczema involves.
The Second Stressor: Chlorine and Chloramine
Hard water is only half of what leaves the showerhead. The same water carries a disinfectant, free chlorine or chloramine, and it acts on the barrier directly. In a controlled study, free chlorine at 0.5 milligrams per liter, a level found in ordinary tap water, significantly reduced the water-holding capacity of the outer skin in people with atopic, eczema-prone skin (Seki et al., 2003). Healthy skin tolerated more before the same effect appeared, and that is the point: the more reactive the skin, the lower the threshold.
The evidence here deserves a precise reading. In the infant cohort above, hard water was a significant risk factor on its own, while chlorine on its own was not, though the two together raised the risk more than either alone (Perkin et al., 2016). What the Water-Softener Trial Taught Us
If hard water worsens eczema, then softening the water should help. Researchers tested exactly that. In a well-run trial, 336 children with established eczema, all living in hard-water areas, were given whole-house water softeners for twelve weeks. The softeners removed the minerals, but the eczema did not measurably improve (Thomas et al., 2011).
That result looks, at first, like a contradiction: the epidemiology says hard water raises the risk of eczema, yet removing the minerals did nothing for it. Two things resolve the tension. First, the studies ask different questions. The population data is about who develops eczema, often in early life, while the trial was about treating eczema that was already established, and established disease carries its own momentum that a single change to the water rarely reverses in twelve weeks. Second, the softener only addressed half of the water: it removed the minerals but left the chlorine and chloramine completely in place, and it traded the calcium for sodium. It treated one stressor and ignored the other. So the trial doesn't show that water is irrelevant. It shows that removing the minerals alone, after eczema has set in, isn't enough, which points straight at the chemistry it left behind. What Water Is Best for Eczema?
The evidence points toward water with the lowest total stressor load, which is not the same as soft water. A softener cuts minerals but adds sodium and leaves the disinfectants untouched, and the softener trial suggests that trade alone does little. The variables most tied to the eczema-prone barrier are the disinfectant chemistry that Seki measured and the calcium-soap deposit that Warren measured.
